All the time. The waitresses don't seem to mind, they just check with the chef first.

There is a local breakfast restaurant that I go to and since I know the owners, I can order a couple tweaks to items on the menu. The only place I've ever done this where I didn't know the place was In-And-Out burger where is it's pretty notorious about their off the menu items.

I never have. But when I was in Spain they had nothing vegetarian on the menu so my group were about to leave (as we had a few vegetarians with us), the chef came out and made a vegetarian paella especially for my table which was so lovely and wonderful customer service!
