You asked this before and the answer is still the same. The very first time you got paid and nothing was withheld, you should have realized you were considered a contractor. (Stop saying you didn't know!) So yes, you MUST file, even if this is your only income. You would use the 1040 with schedule C-EZ (not C3, there is no C3 form) and schedule SE.
