When I last made French onion soup, I simmered it without a cover.

I believe it means lid off. I haven't sauted or caramelised onions for that long before but I've always had lid off for around 30 minutes. The guide via the link provided also appears to suggest lid off.

You can do it either way, as long as you cook the onions very slowly and don't burn them. Depending on your stove heat levels and the pot, you may need to cover and uncover during the cooking process. In other words, it's not all that important how you achieve the desired result as long as you do that.
