Eardrums that are ruptured by an acute infection or by baro-trauma (sudden excess pressure) will often seal over by themselves. But in more chronic cases the perforation may be permanent. Sometimes that may be beneficial, at least for a time. That is the concept of PE tubes. But perforated ear drums can be a detriment, too. Sometimes recurrent infections. Inadvisable to go under water. So those that don't heal are often patched, a surgical procedure. Not as easy as patching a tire/tyre.
