If they're in a hospital, chance is that they're a medical doctor; otherwise, you don't know unless you ask them.

Depends where they work. You also can check at the end of their name.

Medical doctors usually wear those white gowns when they're working. A scientist would wear one, but only if they're in the lab or something. So generally, I think they're attire at work is the key. If they are out of work, I think that the listener would be confused, so they would add detail to their job themselves...
