Wow, your sound so great and romantic, hee hee!!! If I were you I will bring him to a place where both of you see each other for the first time or fall in love :D Or celebrate a surprise party for him.

First thought is Walnut Room at Macy*s they do not take reservations and it is very very busy. I happen to love Garfield Park Conservatory. They are still doing repairs form the hail storm almost 2 years ago but it is a very beautiful spot none the less. The Italian Village has an exceptional restaurant on the second floor. Small alcoves hold tables for more privacy. Prices are not outlandish. Just a stroll down Michigan Ave this time of year is beautiful.

Well, I am a guy and I see three girls have answered these questions which sound great. First, let me tell you what I would have done for my gf. I would have done things she likes shopping is one of them and I hate shopping. 1. Gotten a getaway weekend in SF. 2. I would take her out to shopping in Union Square and be with her while she shopped. 3. Find few great restaurants in SF (whichever type food she likes) by the water. 4. Since its December, I would have taken her out for ice skating or something similar that she would love. 5. My gift would probably be, few love notes hand written on leaves inside a bouquet, whichever chocolate she likes, a beautiful necklace and earrings (probably with a heart shape or something very nice), and I try to surprise her with she would never expect. One of the things I can think of top of my head would be calling her loved ones (family, friends) at the getaway for one altogether dinner. (this can get very expensive). 6. Buy a CD/DVD of one of the favorite music/movie she likes. and more. Thats all I could think of top my head. -------------------- If it was my gf, I would expect nothing in return except a promise from her to love me the way she does now even after 50 years. And, God forbid if we ever drift apart, we would think about this moment and give our relationship a try one more time. As for a gift, whatever you think he likes. My gf knew how much I like to do adventurous activities like snorkeling, hiking, sky diving, hang gliding etc. Whichever activity "with her" would be memorable and a great gift. I hope my ideas gave you a good path forward.
