it means theres coal miners everywhere eg: the sewer is crawling with rats

So the term 'Crawling with Coal Miners' is referring to District 12 having a lot of people, in this case Coal Miners, roaming the streets. Because District 12 is known for having coal, the people there are also known for working to get it. Therefore having hunched backs, swollen knuckles etc. It says that these people are on their way to work and also refers to them being sick of working and have given up trying to change their lives:) Hope this helps!
