Only if you reported the injury at the time if happened. If not, they are going to say it happened elsewhere. It is shady, but that's what they'll try. If you are injured while on the job, your employer is responsible for your injuries.

Did you report it immediately to your supervisor? If so, then it will be covered under worker's comp. Possibly, however, not by Walmart. Who is your actual employer? You said you're a temporary employee--the responsible party is the party who actually pays your wages. If you are paid directly by Walmart, then they are responsible. If you're working for an employment agency, it's on their dime.

Yes, you should be. Contact your HR department to find out who your insurance carrier is.

As long as the injury was sustained in the course of your employment, AND you didn't violate any state statutes what would disqualify you from coverage, yes. ALL employees are covered - part time, full time, temporary, etc.
