Employers can have a variety of reactions to employees calling at the last minute to cancel no matter what the excuse and how valid the excuse is. You need to go in and have a heart to heart visit with the boss and apologize and let him know this was an unavoidable issue. This is one of those times where you making up an excuse places you in a worse light than if you had told the truth. You see an "transportation problem" implies you were careless in your planning. An honest medical issue that comes up at the last minute does not imply you were careless or did not plan well - it is one of those things that happens. You do not need to go into detail on the meds or the condition that is being treated - he does not need to know that. All you need to say is that "I have some prescription meds that made me feel ill and I knew I should not come in." No boss unless they are completely heartless is going to give you grief over that.
