it sounds like you had something done that required authorization to make sure it was covered and you didn't do it. so they won't pay

Yes. You either didn't or they didn't properly record that you did, PRIOR to having the procedure, call to get approval to have that procedure done. So, they cut your benefits that they normally would have provided because you didn't follow the proper steps. If that's not the case, then you refer to your notes and let them know who you talked to and when to get authorization for said procedure.

Because you didn't get authorization in advance from the insurance company, they won't pay as much of the bill, so you will have to pay more of it.
