First thing to check is that it is picking up the electric current efficiently. Have you tried cleaning it? By this I mean removing the body, blowing the dust out of the motor, adding a drop of light sewing machine oil to the gears, bearings etc. Rub all of the track over with fine emery cloth to remove any tarnishing - remembering to remove any dust afterwards. The same can be done with the tyres of the wheels that pick up the current. The grinding noise might be dirt or grit in the gearing, although it might be that the gears are getting worn out. You don't say what type of train set it is - maker, model/catalogue number etc - but if it is a steam locomotive, check out the connecting rods, valve gear etc for lubrication, wear and damage.
