My advice is to call them a soon as possible and explain your absence. If you really believe you need to terminate your employment with them offer them a polite apology for not being able to give them notice but due to the nature of your situation you are not able to meet that type of obligation at this time. I don't think they will hold it against you considering what is going on in your life right now. If they do, then it's probably not a place you really want to be working for in the first place. I have been pretty lucky with my employers over the years. Most of them have understood that family comes first.

Oh, I suggest you to call earlier so that your manager could at least find another person to replace you before your shift starts.

It would be much better if you call her today so she can cover your shift. Leaving her stuck is really wrong. Just tell her the truth, that Gma is ill and you need to stay with her. They may hold your job until you are able to go back.
