If you don't have it in place by March 31, 2014 through the end of the year, you'll pay the higher of 1% of your income or $95.

If you are not insured, if you are not exempt, if you dont have government program, you may have a tax penalty if you file a return and have a refund. The IRS will take the penalty from your refund. They cannot collect it any other way. You have until March 31 to get insured because your tax return is due in April.

Nothing immediately. If you can't afford insurance, then nothing ever. If you can afford insurance, but you decide not to get it, then you'll get a smaller 2014 tax refund (when you get your 2014 tax refund in 2015).
