With metal fatigue, it is possible for some old parts of break. Planes generally have regular safety inspections and not much gets past, unless it is perfect. It can happen, but is NOT very likely

Pilot error causing the aircraft to encounter conditions beyond it's design limitations. There's nothing wrong with the machines. Only the operators. It's possible to hit ANYTHING hard enough to break it. Even an anvil!!
