No-one is obliged to give a reference - so there is nothing you can do about it if she doesn't do it. The NHS would, presumably, ask you to provide someone else as a referee instead.

The NHS will approach the people who were on your CV when you applied. I started my most recent job with 2 other people (company B). One of them only stayed 3 weeks, as he had been for an interview with another company (A) a month before he was was interviewed by B and was offered that job. B just let him go, with 2 days notice, as A was offering the same money, for less hours, as it wasn't worth B getting all upset and resentful.
